on July 21, 2005. PRIZE/ODDS: On or about July 25, 2005, the Sweepstakes Administrator, an independent judging organization, will conduct a random drawing from among all
eligible entries received to award one Grand Prize: a trip for two people to winner’s choice of either New York, NY, Los Angeles, CA, or Miami, FL, to
receive Captique™ injectable gel (hyaluronic acid) and either CosmoPlast or CosmoDerm (injectable collagen) treatments. The prize consists of round trip
coach air travel from a major commercial airport near winner's residence (as determined by Sponsor), standard double-occupancy hotel accommodations for two
nights, Captique™ injectable gel (hyaluronic acid) and either CosmoPlast or CosmoDerm (injectable collagen) treatments for winner and guest, $500 spa services
for winner and guest, and ground transportation or use of a rental car (subject to all car rental agency qualifications and restrictions). Expenses not
specified are winner's responsibility. Approximate retail value (“ARV”) $5,000. Actual value will be determined by winner's residence and seasonal rates.
Fulfillment of the prize is subject to all terms, conditions and medical requirements outlined in the participating medical professional’s service agreement.
Travel and accommodations are subject to availability and certain restrictions. Winner and guest must travel at the same time on the same itinerary.
Trip must be completed by January 31, 2007 or prize will be forfeited in its entirety. Travelers are responsible for any travel insurance and for all trip
expenses not specified. Winner or winner’s guest must be 25 years of age or older and must have a valid U.S. driver’s license in his/her state of residency,
appropriate motor vehicle insurance, and a major credit card to secure the rental car. Optional services or items such as refueling or loss or damage waiver
(LDW) are extra.
Odds of winning will be determined by the number of eligible entries received. Potential winner will be notified by e-mail, phone, postal mail and/or
express mail at Sponsor’s sole discretion, and will be required to complete an affidavit of eligibility, liability and (where legal) publicity release,
which must be returned within time period specified in notification letter or an alternate winner may be selected. Inability to contact the winner, the
return of the prize notification as undeliverable, or winner’s being found to be ineligible or not in compliance with these Official Rules will result in
the prize being forfeited and awarded to an alternate winner selected at random from the remaining eligible entries. Taxes on prize and all expenses
